Use the below form to submit reports you need in as much detail as possible with at least 2 days notice, preferably a week in advance.

Please use the following terminology to describe your data needs:

Member - ONLY describes people that have registered as a member. If someone is a part participant, but recently registered as a member, they are then a NEW member!
On-going Program Participants - people who may or may not be members that are participating in our weekly adaptive sports programming
Wellness Participants - people who may or may not be members participating in Wellness (including Squad) events
Output - quantitative; e.g., how many programs we had, how many athletes we served.
Impact - qualitative; e.g., survey responses. currently there is no impact data in Salesforce
