Please complete the information below. The instructor will provide a webinar or facilitate a wellness group for Sportable. The instructor shall be an independent contractor during the period of performance under this Agreement and not an employee or agent of Sportable. Upon submission of the W9 form and an invoice for $130 emailed to caitlyn@sportable.org, the presenter will be paid within 30 days. As an independent contractor, the presenter shall be responsible for the payment of any taxes due on any monies received. This Agreement shall be governed, interpreted and construed in accordance with the laws of the Commonwealth of Virginia.
